NOTE* The application must be completed by the PI of the study (not the supervisor or co-supervisor) on behalf of the student. The applicant must ensure that the application is successfully submitted by receiving the submission receipt in their registered email.
The review process may be delayed without the following documents being uploaded:
- Completed cover letter with SMU's latest logo (Available on the templates for download)*
- Synopsis*
- A Turnitin report with a similarity index of 20% or less will be accepted*
- Clean protocol in PDF*
- Data collection sheet/tool*
- Departmental letter (only the school of medicine)*
- Facility permission letter*
- Completed signature page*
- The school committee recommendation letter OR approval letter to SMUREC for review and approval (only for degree purposes) + revised protocol
- Completed CV (for Independent Research only)
At this moment, no outstanding documents will be accepted via email or submitted manually.
